Your completed application should include the following items:

1. A completed application form. Download here for a Microsoft Word version or here for a PDF versionor email Dr. Hoese for a digital copy of the application  
2. Vitae or resume
3. Transcripts of high school & all college work (unofficial copies are acceptable)
4. 1-page statement of career plans and description why you want to be a SCERP scholar
5. 1-page statement of past experiences providing preparation for SCERP
6. Letters of Recommendation (2) (letters can be sent via email to Dr. Hoese)
